In a case where the public authority fails to provide information or refuses to provide the same within 30 days you can directly move the Information Commission (State/Central, as the case be) and file a complaint under Section 18 of the RTI Act.
In such a case there would be no need to file the first appeal, though you are free to make a representation to the departmental (1st) appellate authority.

You can not ask questions in a RTI application. You can seek any information, inspect files, obtain documents/material,obtain samples, etc. through RTI; but you can nor ask questions.
While drafting an application, you should take case that your information request is not formatted in the form of "Question."
So instead of asking, "Why did the department took the decision to....etc." you can rephrase your request to, "Provide a certified copy of complete file notings, letter(s), documents, e-mails, and documents conating the reasoning and/or on the basis of which the department took the decision to ......"
